There was an insect on a leaf (shown in last photo), but I don’t believe it had anything to do with the holes. The disease is called Shot Hole disease, caused by the bacterium Xanthomonas arboricola pv. pruni. according to http://agri.nv.gov/PLANT_PATHOLOGY/2006/ShotHoleDisease.pdf
